For millions of passengers, reaching cruising altitude means settling into a seat, opening a laptop or waiting for the drink cart. Pilots and flight attendants experience something different. Their workplace exposes them to an invisible environmental factor that most travellers rarely think about: cosmic ionizing radiation.

A new study published in JAMA Internal Medicine on August 17, 2026, has renewed attention on this issue. Researchers found higher mortality from cancers they classified as potentially radiation-related among pilots and flight attendants than among people in other occupations.

The findings raise important occupational-health questions. However, the researchers did not prove that cosmic radiation encountered during flights caused those cancer deaths.

What Did the New Aircrew Cancer Study Find?

The results showed higher risk-adjusted mortality from radiation-related cancers among both pilots and flight attendants compared with other workers.

Commercial aircrew work in an unusual environment. Unlike most employees, they can spend hundreds of hours every year several kilometres above Earth’s surface. At those altitudes, Earth’s atmosphere provides less protection from cosmic radiation.

Mortality records can reveal patterns across large groups, but they cannot identify the exact cause of an individual person’s cancer. That distinction matters when interpreting headlines about the research.

Why Does Radiation Increase at Cruising Altitude?

Energetic particles from space and solar activity produce cosmic radiation. Earth’s atmosphere shields people from much of that radiation before it reaches ground level.

A commercial aircraft cruising around 35,000 feet travels above a substantial part of that protective atmosphere.

Health Canada says cosmic-radiation intensity at commercial aircraft altitudes can reach approximately 100 times the intensity at ground level. Altitude, latitude, flight duration and solar conditions all affect the dose a person receives during a flight.

Earth’s magnetic field also changes exposure levels. It provides stronger protection near the equator and weaker protection near the poles. Because of this difference, high-latitude routes can expose aircrew to more cosmic radiation than comparable routes closer to the equator.

Frequent Flyers and Aircrew Face Very Different Exposure

The phrase “radiation at 35,000 feet” can sound alarming to anyone who travels by plane. But occasional passengers and professional aircrew accumulate very different levels of exposure.

Health Canada considers cosmic-radiation exposure very small for occasional flyers. Pilots and flight attendants face a different situation because they spend a significant part of their working lives aboard aircraft.

Health Canada estimates that a person would need to take approximately 100 one-way flights between Toronto and Vancouver to receive a cosmic-radiation dose comparable to the natural background radiation that an average person receives from other sources during one year.

Aircrew can accumulate exposure flight after flight for years or even decades. Health Canada notes that frequent flyers such as aircrew can receive annual doses comparable to, or even higher than, some radiation workers in ground-based industries.

Canada Has Studied Aircrew Radiation Exposure Before

Canadian authorities have studied cosmic-radiation exposure among pilots and flight attendants for years.

The Canadian Aircrew Radiation Environment Study measured radiation doses across different flight routes. Transport Canada reported estimated annual ambient dose equivalents of approximately 1 to 6 millisieverts (mSv) for selected aircrew during the study period.

The research also showed a clear pattern. Higher altitude, higher latitude and longer flight time increased exposure.

Transport Canada later recommended that Canadian air operators assess cosmic radiation as an occupational exposure when employees could receive more than 1 mSv annually and take appropriate steps to manage that exposure.

Some Flight Routes Create More Exposure Than Others

Two pilots can log similar annual flight hours and still accumulate different cosmic-radiation doses.

A long-haul aircraft travelling across northern latitudes at high altitude can expose its crew to more radiation than a shorter flight operating farther south at a lower altitude.

Transport Canada’s 2026 Aeronautical Information Manual gives one useful example. A pilot who flies 500 hours per year at approximately 35,000 feet between 60 and 90 degrees latitude could receive an estimated annual exposure of about 1.73 mSv.

For this reason, researchers and aviation authorities consider route, altitude, latitude and accumulated flight time when estimating occupational exposure rather than simply counting flights.

Cosmic Radiation May Not Tell the Whole Story

Aircrew encounter several unusual working conditions at the same time, which makes the new study harder to interpret.

Pilots may encounter UVA radiation through cockpit windows. Pilots and flight attendants also work overnight schedules, cross multiple time zones and repeatedly disrupt their normal sleep patterns.

Those schedules can disturb the body’s circadian rhythm, which helps regulate sleep, hormones and many other biological processes.

An accompanying commentary in JAMA Internal Medicine highlighted several potential occupational exposures, including cosmic ionizing radiation, cockpit UVA exposure and circadian disruption.

Researchers therefore face a difficult task when they try to isolate the effect of cosmic radiation from other characteristics of aviation work.

⚠️ Association does not prove causation:
Researchers found higher cancer mortality among aircrew, but they did not measure each person’s lifetime cosmic-radiation exposure. The study therefore cannot prove that radiation caused an individual cancer.

Why Does This Study Still Matter?

Researchers do not need to prove causation in a single study to identify an occupational-health issue that deserves further investigation.

This research draws strength from the large number of mortality records the researchers analyzed. That scale allowed them to identify patterns that smaller studies might miss.

The results also give researchers another reason to study cumulative exposure more precisely. Future studies could combine individual flight histories, route-specific radiation estimates and long-term health records.

That approach could help researchers answer the question that mortality records alone cannot settle: does cancer risk rise as a person’s cumulative cosmic-radiation dose increases after researchers account for other occupational factors?

What Does Health Canada Say About the Cancer Risk?

Health Canada takes a measured approach to the potential health effects of cosmic radiation from air travel.

At the exposure levels associated with flying, Health Canada says radiation may cause a very slight increase in the chance of developing cancer many years or decades later.

That wording matters. Health Canada recognizes that ionizing radiation carries risk, but it does not suggest that taking a flight — or working aboard an aircraft — means a person will develop cancer.

The amount of radiation a person accumulates matters, which explains why occupational-health experts focus on long-term exposure among aircrew.

What Should Pregnant Aircrew Know?

Radiation-protection guidance pays particular attention to pregnancy because an unborn child can have greater sensitivity to ionizing radiation.

Health Canada explains that radiation-protection authorities recommend limiting additional exposure to an unborn child because scientists still face uncertainty about very low radiation doses.

Health Canada’s air-travel guidance says approximately 200 hours of flying would correspond to its referenced exposure level, while also noting that experts expect no observable health effects at that dose.

Transport Canada has also encouraged employers to inform workers about pregnancy-related exposure and adjust work schedules when necessary.

Pregnant pilots or flight attendants who have concerns about their individual exposure can discuss their flight schedules with their employer’s occupational-health program and an appropriate healthcare professional.

Should Passengers Worry About Flying?

The new research does not show that ordinary passengers should avoid air travel because of cancer fears.

Health Canada considers exposure very small for occasional flyers. Pilots and flight attendants present a different occupational-health question because they can spend thousands of hours at altitude throughout their careers.

For occasional travellers, the study does not provide evidence that a vacation or business flight creates a major cancer danger.

The research instead raises questions about people who spend years working inside aircraft cabins and cockpits.

What Question Does the Aviation Industry Face Next?

The findings add new evidence to an issue that researchers and aviation authorities have examined for years: how should airlines manage an environmental exposure that workers slowly accumulate throughout their careers?

Airlines cannot switch off cosmic radiation. Conventional commercial aircraft also cannot completely shield crews from it. Instead, operators can estimate exposure by considering flight time, routes, altitude and latitude.

Better long-term exposure records could give Canadian pilots and flight attendants a clearer picture of the doses they accumulate throughout their careers.

The latest study does not settle the question of whether cosmic radiation causes the higher cancer mortality researchers observed among aircrew. It does, however, give scientists another reason to investigate the relationship carefully — without turning an occupational-health concern into unnecessary fear for occasional passengers.

Bottom line: Pilots and flight attendants encounter more cosmic radiation than people on the ground, and the new study found higher mortality from cancers that researchers classified as radiation-related. Researchers have not proved that cosmic radiation caused those deaths. Better measurements of cumulative exposure could help answer that question.
